The only thing that we can say about these cases is that there is an imbalance in the sympathetic nervous system, the part of the body that regulates and controls how much we sweat. It increases sweating to cool us down when we need to e.g. during exercise, and allows us to sweat only as much as we need to.
In someone with hyperhidrosis, there is an imbalance in this mechanism. Sometimes it is passed genetically but most people have no idea why they have the condition.
So what are the treatment options?
Clinical Antiperspirant
The first thing to try is a medical grade antiperspirant. Unlike store varieties, this can be used on any part of the body and not just the underarms.
Apply it to the palms and the soles of the feet before bedtime and wash it off in the morning. Some people may only need as little as one application per week.
The downside to these stronger antiperspirants is that although they work better they also give more irritation.
Iontophoresis
This technique is especially suited to excessive sweating of the hands and feet.
Two shallow trays are filled with water – the feet are placed in one and the hands in the other.
A low electric current is then passed for around 20 minutes. This is not painful but some sensation is felt, said to be a “tingling” feeling.
This has the effect of blocking the sweat ducts for around 2 to 3 weeks at a time. No chemicals are introduced into the body (unlike aluminium chloride in the case of antiperspirants). If regular treatment is needed, it can be wise to invest in a home kit.
Botulinum Toxin
You may have heard of this being used to smooth out wrinkles but it has long been used in the medical profession.
Injecting into the hands and feet can stop sweating for anywhere between four and 13 months. However, it is not cheap and many doctors will charge around $1,500 for the hands alone.
Additionally, many doctors will not treat the soles of the feet and there may be some who also refuse to treat the hands, as the consequences of using too much on the hands would severely affect simple everyday tasks such as picking up objects.
Drugs
The only drug that is effective for excessive sweating are anticholinergics. These work by reducing all bodily secretions.
So not only do they stop your hands and feet sweating but also the rest of your body plus your tears and saliva production.
They are only recommended for general excessive sweating all over the body so, unless this includes you, I would avoid these drugs as the side effects can be severe.
ETS Surgery
This surgery involves cutting the nerves that carry messages to the sweat glands. Cut the nerves and the message “to sweat” will never be received.
This type of surgery is highly controversial. First of all, it is rarely used for plantar hyperhidrosis due to the risk of sexual dysfunction. Secondly, complications are common and often include compensatory sweating elsewhere on the body such as the head or the trunk.
Natural Approaches
Because nobody truly understands the underlying cause of hyperhidrosis and how the imbalance in the sympathetic nervous system arises in the first place, the medical establishment is usually focussed on the symptoms of this condition, the sweating.
